Privacy Policy

1. We respect your privacy  

1.1. Lighthouse Physiotherapy and Health Collective (LPHC) (ABN 28 678 927 814)is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information. We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The APPs govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information. A copy of the Australian Privacy Principles may be obtained from the website of The Office of the Australian Information Commissioner at www.aoic.gov.au.  

1.2. We may change our Privacy Policy from time to time by publishing changes to it on our website. We encourage you to check our website periodically to ensure that you are aware of our current Privacy Policy. 

1.3. "Personal information" is information we hold which is identifiable as being about you.  

2. Collection of personal information  

2.1. LHPC will receive and store personal information you enter onto our website, provided to us directly or given to us in other forms such as the patient registration form. Such as financial and related information such as your payment details, government identifiers and health insurance details.  

2.2. You may provide basic information such as your name, phone number, address and email address to enable us to send information, provide updates and process your product or service order. We may collect additional information at other times, including but not limited to, when you provide feedback, when you provide information about your personal or business affairs, change your content or email preference, respond to surveys and/or promotions, provide financial or credit card information, or communicate with our customer support.  

2.3. LPHC may collect sensitive information such as health conditions, ethnic origin, medical history, medications, allergies, adverse events, treatments, relevant social and family histories and other medical records. LPHC may collect Information for safe provision of goods or services such as details of the products and services we have provided to you or that you have enquired about, including any additional information necessary to deliver those products and services and respond to your enquiries and information you provide to us. 

2.3. Additionally, we may also collect any other information you provide while interacting with us. Such as information relating to you that you provide to us directly through our website or app or indirectly through your use of our website or app or online presence or through other websites or accounts from which you permit us to collect information. 

3. How we collect your personal information  

3.1. LHPC collects personal information from you in a variety of ways, including when you interact with us electronically (through correspondence, email, or when you share information with us from other social applications, services or websites) or in person and when we provide our services to you. We don’t guarantee website links or policy of authorised third parties 

3.2. Personal information may be collected through  information provided by your health fund; or during the course of providing healthcare services. When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.  

4. Use of your personal information  

4.1. LPHC may use personal information collected from you to provide you with information, updates and our services. We may also make you aware of new and additional products, services and opportunities available to you. We may use your personal information to improve our products and services and better understand your needs. .  

4.2. LHPC may contact you for direct marketing communications and information about affiliated services and products. This communication may be by a variety of measures including, but not limited to telephone, email, sms or mail. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing 

 

5. Disclosure of your personal information  

5.1. We may disclose your personal information to any of our employees, officers, insurers, professional advisers, agents, suppliers or subcontractors as reasonably necessary for the purposes set out in this Policy. Personal information is only supplied to a third party when it is required for the delivery of our services. Including providers for the operation of our websites and/or our business or in connection with providing our products and services to you.  

5.2. We may from time to time need to disclose personal information to comply with a legal requirement, such as a law, regulation, court order, subpoena, warrant, in the course of a legal proceeding or in response to a law enforcement agency request.  

5.3. LPHC may also use your information for payment systems operators (eg merchants receiving card payments) 

5.3. We may also use your personal information to protect the copyright, trademarks, legal rights, property or safety of LPHC its customers or third parties if required.  

5.5. If there is a change of control in our business or a sale or transfer of business assets, we reserve the right to transfer to the extent permissible at law our user databases, together with any personal information and non-personal information contained in those databases. This information may be disclosed to a potential purchaser under an agreement to maintain confidentiality. We would seek to only disclose information in good faith and where required by any of the above circumstances.  

5.6. By providing us with personal information, you consent to the terms of this Privacy Policy and the types of disclosure covered by this Policy. Where we disclose your personal information to third parties, we will request that the third party follow this Policy regarding handling your personal information.  

 

6. Security of your personal information  

6.1. LPHC is committed to ensuring that the information you provide to us is secure. In order to prevent unauthorised access or disclosure, we take reasonable steps to protect your personal information from misuse, interference and loss, as well as unauthorised access, modification or disclosure and we use a number of physical, administrative, personnel or technical measures to protect your personal information.  

6.2. The transmission and exchange of information is carried out at your own risk. We cannot guarantee the security of any information that you transmit to us, or receive from us.  

6.3. We may hold your personal information in either electronic or hard copy form. When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years. 

 

7. Access to your personal information  

7.1. You may request details of personal information that we hold about you in accordance with the provisions of the Privacy Act 1988. In order to protect your Personal Information, we may require identification from you before releasing the requested information. LHPC will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information. If you would like a copy of the information, which we hold about you or believe that any information we hold on you is inaccurate, out of date, incomplete, irrelevant, or misleading, please email us at info@lighthousephysiohc.com.  

7.2. We reserve the right to refuse to provide you with information that we hold about you, in certain circumstances set out in the Privacy Act.  

8. Maintaining the Quality of your Personal Information 

8.1. At LPHC it is an important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up to date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you. 

9. Complaints about privacy  

9.1. If you think we have breached the Privacy Act, or you wish to make a complaint about the way we have handled your personal information, please send in details of your complaint to info@lighthousephysiohc.com. We take complaints very seriously and will respond to you regarding your complaint within a reasonable period of time. If you think that we have failed to resolve the complaint satisfactorily, we will provide you with information about the further steps you can take.

10. Website  

10.1. We may collect personal information about you when you use and access our website. We may record certain information about your use of our website, such as which pages you visit, the time and date of your visit and the internet protocol address assigned to your computer. This information is used in an aggregated manner to analyse how people use our site, such that we can improve our service.  

10.2. Cookies. We may from time to time use cookies on our website. Cookies are very small files which a website uses to identify you when you come back to the site and to store details about your use of the site. They enable the entity that put the cookie on your device to recognise you across different websites, services, devices and/or browsing sessions. You can disable cookies through your internet browser but our websites may not work as intended for you if you do so. We may also use cookies to enable us to collect data that may include personal information. We will handle any personal information collected by cookies in the same way that we handle all other personal information as described in this Privacy Policy. In addition, cookies may be used to serve relevant ads to website visitors through third party services. 

10.3. Links. Our website may from time to time have links to other websites not owned or controlled by us. Links to third party websites do not constitute sponsorship or endorsement or approval of these websites. Please be aware that LHPC is not responsible for the privacy practises of other such websites. We suggest our users to be aware, when they leave our website, to read the privacy statements of each website that collects personal identifiable information.
